Quick answer: PGI Chandigarh (PGIMER) does not take online appointments with a chosen doctor. What you can do online is a free Online Pre-Registration for a department on pgimer.edu.in. You get a Pre-Registration Number, which you show at the registration counter on your chosen date: 8:00 to 11:00 AM, Monday to Friday, or 8:00 to 10:30 AM on Saturdays and gazetted holidays.
